This checklist is to help you make sure you have all the needed documents for your application.
Students enrolled in a NSW Government School
Students in a NSW Government School should have the following documents to send in with their completed application:
- Completed Single Course Provision Application form
- Include category for enrolment and supporting material
- All pages completed
- Signed by Principal, Supervisor, Parent/Guardian and Student
- Languages Declaration for Stage 6 students (if applicable)
- Unforeseen and Unique Circumstances Application form (if applicable)
Note: An inter school journal will be created by Finigan school of Distance Education if the enrolment application is approved.
Students enrolled in a Non Government School
Students in a Non Government School should have the following documents to send in with their completed application:
- Completed Single Course Provision Application form
- Include category for enrolment and supporting material
- All pages completed
- Signed by Principal, Supervisor, Parent/Guardian and student
- Languages Declaration for Stage 6 students (if applicable)
- Unforeseen and Unique Circumstances Application form (if applicable)
Tax invoice
A Tax Invoice will be created by the Education Centre if the enrolment application is approved.
